Student Application Form
may have to be printed out and completed by hand. (Some computers
will not print out information entered on the screen.)

For
payment instructions and list of all documentation to be submitted,
print out and complete the Registration Checklist.

Please apply at least six weeks prior to the date you wish to start. Mail
by postal service or fax all documents and payment. Do not e-mail.
