VACATION STUDY APPLICATION INSTRUCTIONS

1. Print out and complete the Vacation Study Application Form.
2. $500 is due with the application. This payment includes registration fee, SEVIS fee and tuition deposit. For payment instructions and list of all documentation to be submitted, print out and complete the Vacation Study Registration Checklist.
3. Please apply at least six weeks prior to the date you wish to start. Email, fax or mail by postal service all documents and payment. (If documents are emailed or faxed, please bring the original certified bank letter to GGLS on the first day of class.)

To help us process your application quickly, please check and include the items on the following checklist.
Vacation Study Application Form. Be sure to sign the form.
Declaration of Guardianship form, if the student is 16 or 17 years old.
Certified bank letter(s) from sponsor.
Proof of medical insurance. If not included, then once you have purchased a policy, please fax: 1-408-374-9429 or email: vt@goldengatelanguage.com.
$500 payment or credit card information.
Homestay Application, question #16.
Airport Pick-up Application, question #17.
Vacation Study Registration Checklist, this form.
Payment due in U.S. dollars. Please indicate the payment method. This is not a secure site. Do not e-mail!
